PURPOSE
The Lake Country Community Health and Well-Being Survey invites residents to share their recent experiences with factors that shape health and wellness, including social connections, economic stability, environmental conditions, and access to primary and community care.
Your input will help create a clear, data-driven picture of health and well-being in Lake Country. The results will reflect resident voices and will guide community-driven initiatives and partnerships to create a healthier, more equitable community here in Lake Country.
CONFIDENTIALITY
Note that your answers will be kept strictly confidential, and survey results will be reported collectively. No personal identifiers are collected as the survey is anonymous. The survey meets both FIPPA (Freedom of Information and Protection of Privacy Act) and CPHIR (Confidentiality of Personal and Health Information Regulation requirements).
RMF Clinical Health Strategies is an independent consulting company hired by the District of Lake Country to conduct this survey. INSTRUCTIONS
This survey takes about 15 minutes to complete online. To be eligible to complete, you must be a Lake Country resident and at least 18 years of age.
If completing online, use the Next and Previous buttons to move through the survey. You may exit the survey at any time by closing your browser. When you return to the survey link, your answers will still be there. Once you submit your responses, you will not be able to return to the survey.
